We are sorry that your shipment has been damaged.  In order to file a claim with the carrier we need the following information:

 

1) Your name, address, phone, and email.

2) Where the shipment was sent to.

3) Date of shipment, Shipment ID# if you have your receipt.

4) Carrier used for shipment.

5) Pictures of damaged box and items.

6) Invoice showing value of items damaged.
